Narrative
An employee was unloading materials onto a roof when the employee slipped and fell off the roof. During the fall to the ground, the employee's head hit a trailer, resulting in a fractured skull and injuries to the nose, mouth, teeth, jaw, ear, and left shoulder. The employee lost consciousness and was hospitalized. Fall protection was not worn at the time of the incident.
