An employee was attempting to retrieve a small stack of pallets using a front rider pallet jack. As the employee was reversing to exit the rack storing the pallets, the pallet jack s front lift control box caught on a rack beam. The employee s left index finger was caught between the lift box and the rack beam, and the fingertip was amputated.
