Narrative

An employee was doing maintenance on a circular head saw blade splitter. The employee was straightening the splitter with an adjustable wrench and cheater pipe extension. The wrench slipped off the splitter, causing the employee to fall backwards onto a platform and suffer a dislocated/fractured right ankle.
