Narrative

An employee was on the production floor cleaning a machine with a metal rod. The metal rod got caught and was pulled by the gear of the machine causing the handle of the rod to press the employee's fingers against the edge of the area being cleaned. The employee's index, middle, and ring fingers were amputated.
