Narrative

An employee was reversing the lock out/tag out while on a pumpjack, removing the chain and binder from the unit. The employee slipped and fell onto an angle iron on the outer side of the concrete base. The employee sustained injuries to his right side, including his right hip and elbow, as well as transverse process fractures in two locations.
