Narrative

On July 22, 2020, at approximately 12:15 PM, the injured employee had been removing a jammed bag from a packaging machine. A supervisor found the injured employee on the floor, unresponsive. The employee had been injured by the bag carriage device on the packer machine. He was hospitalized with head trauma, lacerations on the left eyebrow and back of the head, two broken ribs on the right side, and multiple fractures to the right side of the jaw requiring surgery.
