Narrative

An employee was sitting on the seat of a forklift when he stuck his arm through the mast of his forklift to scan freight. The cord from his scan gun caught the lever to move the mast up and down, causing the mast to retract and come down on his right arm. As a result, he suffered a right arm amputation.
