Narrative

An employee was preparing steel flats to be loaded using an overhead crane and a hook. He lifted the material about 4 inches up and attempted to put a block under the material with his right hand. The material slipped off the hook and came down on his right hand, crushing and amputating the index finger below the second knuckle.
