Narrative

An employee was driving a forklift forward when she tried to move a dolly's break bar by pushing it with her arm. Her ring and middle fingers were then caught between the overhead guard frame of the forklift and the dolly's break bar, resulting in a ring fingertip amputation and middle finger laceration.
