Narrative

An employee was working on a concrete manufacturing machine. As he was attempting to change out the forms, a forklift pulled up with a new form. He stepped sideways over a pit area where the form would sit, fell approximately eight feet, and struck his head. He was hospitalized for a fractured skull.
