Narrative

An employee was transferring liquid soap from one storage tote to another storage tote. The hose had pressure built in the line and when the nozzle was turned on to allow for liquid release, the liquid forcefully exited the hose and struck the employee's eyes resulting in chemical burns to both eyes.
