Narrative

An employee was cleaning up at the end of the shift and putting small hand tools and extension cords away in a large, steel tool box. The employee was holding the tool box lid open with one hand and putting tools away with the other hand when the lid slammed shut causing the amputation of the employee's left ring fingertip.
