Narrative

An employee was trying to pull a part out of the outfeed end of a thermoformer. The scrap wrapped around the chain, which pulled the scrap backwards and in turn pulled her hand into the scrap shear. The shear actuated and amputated her middle, ring, and little fingers above the knuckle.
