Narrative

On December 15, 2020, an employee was inspecting a palletizer machine and pallet feed when he slipped and his hands contacted the back side of the pallet lift (a stationary forklift) to break his fall. His left index finger was caught between the pallet's gear and the fork, resulting in a fracture and an amputation without bone loss. The employee was hospitalized.
