Narrative

An employee was installing round duct in a clean room ceiling. When he placed his knees on an access hatch door, the latches gave way and the employee fell approximately 10 feet through the opening to the metal floor below. The employee sustained fractured legs and a fractured left shoulder and was hospitalized.
