Narrative

Two employees were hanging a mold in a machine. The injured employee was on the back side of the machine, holding a mold in the center location, while the other employee was adjusting the die heights in order to hold the mold in position. The mold moved and pinched the injured employee's pinky finger between the platen of the machine and the mold, resulting in amputation.
