Narrative

An employee was stepping from a metal grating onto a sloped metal ramp, 2 inches high by 6 inches wide. He slipped on residual graphite dust and fell backwards. The back of his head struck the metal grating and he suffered lacerations and a brain bleed. He was hospitalized.
