An employee was working from a step ladder, trying to place a piece of scrap into a basket on a rack system. There was not sufficient clearance to put the pipe in, so the employee pulled the basket out. The basket fell against the ladder and tipped it; the employee fell about 6 feet to the floor below, and the basket landed on his foot. He suffered a broken foot, leg, and hip.
