Narrative

An employee was operating a forklift when the front tire hit a drain. This caused the lumber that the forklift was carrying to fall off, tilting the forklift forward enough to lift the back tires off the ground. The back tires then slammed back onto the ground. The employee was hospitalized for a fractured back.
