Narrative

An employee was unloading a small bundle of lumber from a truck using a forklift. As he backed away from the truck, the forklift's rear wheel rolled into a curb. The resulting jolt caused the top of the employee's head to strike the forklift's roll-over protective cage. He suffered a laceration to the top of the head.
