Narrative

An employee was filling up a boom lift with a 5-gallon diesel can. He fell from the top of the boom lift's wheel to the ground and his face hit the fuel can. He suffered a cut above his left eyebrow and nose from the fuel can, as well as a broken neck vertebra and a contusion to the right hand from the fall.
