Narrative

An employee was attempting to install a cap (a two-part assembly) onto a casing head. The cap was stuck, and as the employee tried to manually thread it onto the casing head, its upper part came down and caught the employee's left thumb. The employee suffered an amputation to the thumb and was hospitalized for surgery.
