Narrative

On April 6, 2021, an employee was operating a forklift and activated the dead man lever in order to avoid a collision. The forklift stopped abruptly and jerked forward, causing the employee's left arm to get caught between the lift and the racking. The employee sustained a laceration to the left forearm.
