Narrative

An employee was working in an elevator that was being built. While stepping from the floor of the elevator (which was raised 4 feet off the floor on sawhorses) to an 8-foot fiberglass ladder, he missed his rung and fell backwards, landing on his shoulder on the floor. He suffered a shoulder injury.
