Narrative

An employee was releasing the pressure from a setting tool. As the employee was screwing a wrench into the burst disc retainer, the retainer nut came free and the pressure escaped from the chamber. Debris struck the employee in the right shoulder and face, leaving foreign material in his eyes and causing contusions to the shoulder and the right side of his face.
