On 05/12/2021, at approximately 8:52 AM, an employee of Petrin Corp. was on an elevated, grated platform at a chemical plant replacing a removable barricade with a hard barricade when he tripped, lost balance, and fell approximately 20 feet. The employee sustained fractures to a hip, the right arm, and the right wrist.
