Narrative

On June 14, 2021, at approximately 11:00 AM, an employee was on a mezzanine handing a tool to a co-worker beneath him. The top rail of the mezzanine's guardrail broke and the employee fell 9 feet to the concrete floor. The employee was hospitalized with fractures to the left hip and left elbow.
