Narrative

An employee was calibrating elevated refrigeration probes while inside a cage about 15 to 20 feet off the ground, chained and padlocked to the forks of a forklift. The forklift backed into a safety bollard. The cage broke free as the chains and padlock snapped. It fell to the ground and the employee suffered a broken back with spinal cord damage.
