Narrative

An employee was clearing jammed material from a pelletizer with a rotating cutter. As he was removing uncut strands of acrylic from the guide tray, his left index finger was pinched between the two feed rolls of the pelletizer. The rotor blade then cut his finger, causing the amputation of the fingertip.
