Narrative

At about 2:00 p.m. on August 26, 2021, an employee was replacing a pulley on a conveyor belt. The employee's left hand was pinched between the pillow block bearing and the base plate, and the employee suffered amputations to the tip of the little finger, half of the ring finger, and half of the middle finger.
