Narrative

An employee was trying to remove an automobile floor mat from a rug beater machine. The side service panel had been removed from the machine so she could diagnose a possible issue with the machine. The mat came free, and her right hand slipped into the path of the machine's chain and sprocket. Her right index finger was amputated at or near the first knuckle.
