Narrative

An employee was removing insulation from the dust filters on the second floor of the warehouse. The employee was climbing an 8-foot step ladder to access the work area when his right foot slipped on a rung, causing him to fall about 5 feet to the ground below. He landed on both knees and sustained fractures to his pelvis and knee.
