Narrative

An employee was standing with a fully loaded walkie pallet jack on the side lift gate of a delivery truck, preparing for a delivery. The lift gate platform broke at the hinges, causing the employee, the pallet jack, and a loaded pallet to fall about 4 feet to the concrete ground. The employee suffered a broken lower right leg.
