Narrative

An employee was putting a chain back on a forklift mast pulley when the chain became taut and crushed his left hand between the pulley and the chain. The employee suffered smashed and lacerated fingers, resulting in amputations of the middle and ring fingers as well as a skin graft to the little finger.
