An employee was positioning a workpiece on the table of a mortise and tenon machine. She pushed the start button with her left hand and her right hand was resting on the workpiece. As the machine actuated, the guard that protects the rotary cutter head crushed the employee's right middle fingertip, resulting in an amputation.
