Narrative

At 2:15 p.m. on January 18, 2022, an employee was shearing 14-gauge alloy steel sheet material on a hydraulic shear. When the machine activated, the plunger of the hydraulic hold-down cylinder came down and crushed his left middle fingertip, causing an amputation.
