Narrative

An employee was driving a powered industrial truck through a product storage aisle. The back of the truck struck a wooden pallet that was sticking out of a rack. The pallet was pushed into another pallet. This caused a board to break off from the pallet and enter the truck, striking the employee in the left side. The employee suffered broken ribs on the left side, as well as a hernia between ribs.
