Narrative

At about 8:30 a.m. on January 27, 2022, an employee was clearing a jam in a machine. While he was kneeling beneath a metal pocket on the machine (about 4 feet high, weighing about 10 pounds) that held parts, the pocket was lifted up off the chain and fell out, striking the employee on the head. He suffered a head laceration, a skull fracture, and internal bleeding and was hospitalized.
