Narrative

An employee was using a powered industrial stacker to move completed pallets of blended product from the manufacturing area to the warehouse. The employee tripped and fell to the floor, inadvertently moving the throttle of the powered industrial stacker and causing it to move forward and crush his right foot. The foot was surgically amputated.
