Narrative

An employee was returning a pallet to his truck after a delivery. While he was adjusting the pallet, which weighed 100-200 pounds, it moved and caught his right little finger against the truck's pallet-securing device. The tip of his little finger was amputated and he suffered a nailbed avulsion.
