Amputations

An employee was moving a stack of pallets into a trailer using an unpowered pallet jack. There were two stacks, side by side, in the trailer. The right stack had three pallets on top that were not perfectly aligned with the rest of the stack. As the employee was pushing the left stack into position next to the right stack, the three pallets on the right stack caused the top three pallets on the left stack to tip over. The employee suffered a half-inch soft tissue amputation to the right middle finger.
