Amputations

An employee drove a skid steer carrying a dump hopper containing demolition debris to a 7-foot-high dumpster. He lifted the dump hopper up so the forks were just above the edge of the dumpster and moved forward so the dumpster edge would support the bottom of the dump hopper. He then left the skid steer and put his hand under the fork to use as leverage so he could push the lever to dump the hopper. When he pushed the lever, the hopper tilted forward, causing the pan by the forks to lift up; then, as the hopper came back, the pan came down on his left middle and ring fingers. He suffered amputations to both fingertips.
