Narrative

At 11:00 a.m. on April 28, 2022, an employee was changing a tire on a trailer. He had jacked up the trailer, removed the tire, and repaired it. As he was trying to put the tire back on, the jack slipped. His left arm was trapped between the tire and the ground, and his forearm was broken.
