On May 2, 2022, an employee was performing maintenance on a de-energized press, which entailed replacing a junction box cover. The employee lost their balance as they were coming down from the work area and began to fall backward. The employee reached out and grabbed the belt that connects the flywheel to the motor. The belt then rotated downward and pinched the employee's left middle finger between the motor pulley and the belt. The fingertip was amputated.
