Narrative

An employee was attempting to seal a leak at the strainer plug located on a pressurized anhydrous ammonia toolbar. The employee was tightening a plug on the strainer using a wrench when the plug failed and a burst of anhydrous ammonia hit the employee in the face and torso. The employee suffered chemical burns to the eyes and mouth as well as the left side of their head, neck, and shoulder.
