An employee had been using a truck-mounted winch system to load or unload a roll-off box. The "D" ring of the reeving system came off the hook that was manufactured on the dumpster, striking the employee in the chest and face. The employee sustained multiple fractures and the loss of an eye, which was surgically removed.
