Narrative

On June 9, 2022, at 10:45 AM, an employee was standing on a trailer deck (tailgate) waiting for the customer to lower an empty poultry cart down using a scissor lift. The cart fell off the lift from 25 feet above the employee and struck the employee's left arm resulting in a broken radius and ulna.
