Narrative

An employee was in the back of a piece of mulching equipment cleaning it. As the employee was raking material off the rotating drum, the rebar-like material that is attached to the drum caught the employee's shirt and pulled him into the machine. The employee suffered three puncture wounds to the chest in the armpit area with a punctured lung. The employee was hospitalized. The machine was not locked or tagged out at the time of the incident.
