Narrative

An employee was standing next to a flatbed truck as it was being loaded with 6-inch ductile iron pipes. As the pipes were being loaded, a pre-fabricated wood chock snapped and struck the employee in the face. The employee suffered a broken nose and a concussion and was hospitalized.
