Narrative

A brush component of an optic grader was running with a cover up for visual cleanliness inspection. An employee was cleaning an area in the brush component's drive system when the towel she was using got caught in the system's chain and sprocket. The towel wrapped her hand and pulled it into the chain and sprocket, resulting in an amputation at the mid-forearm.
